In Iraq it's more of a hand hold and sometimes done with both hands. Gotta remember that Arab men often hold hands as a sign of friendship and we see that in Compound 5 among the HVDs.
Lately our detainee population from Compound 3, particularly the Shi'a and Takfiris have been refusing our shake hands. So we dismiss these patients with the message: "Our handshake is a form of respect and once its returned we will treat you as a patient. Otherwise, we're done for the day."
